Quorum

Meeting is conducted depending upon the availability of committee members minimum 65 percentage of members should take part in the meeting. Less than 65 percentage meeting will be reorganized on later date.

Terms of Reference

Communication is sent individually to all committee members through the Chairperson or coordinator of committee one day before the meeting.

Role of Committee

  • To review the appropriateness of ordering policies of Blood/components.
  • To review the handling and administration of blood and its components
  • To review medical record randomly selected from the list of transfused patients.
  • To monitor and discuss issues related to :
    • Safety of blood from transmissible diseases.
    • Amount of blood / blood products used.
    • Monitor transfusion practices compared to institutional, national or international benchmarks – promotion of safe and effective blood transfusion practice.
    • To review summary report of statistics such as wastage and usage of blood and blood products.
  • Reviews of transfusion reaction (where corrective measures are necessary).
  • To promote continuous medical education in transfusion medicine for hospital staff.
